Tennessee's prisons chief quits after stunning failed execution
Tennessee's prisons chief has resigned after a botched execution left death row inmate Christa Pike on a ventilator in hospital following two doses of lethal drugs.
Strada said in a statement that resigning was “in the best interest of Tennesseans” while an independent review determines what happened.

Pike's lawyers allege problems with IV lines caused the pentobarbital to flow into her body rather than her bloodstream, calling it a systemic failure.
Governor Bill Lee has ordered an independent review and halted all new executions while the investigation into what went wrong is completed.
